Report the death of someone with a learning disability or an autistic person



Anyone can use this online form to report the death of someone who:

To report the death of a child or young person under 18 years of age please visit Child Death Review.

The information you provide when you report a death will be used by the Learning from Lives and deaths, people with a learning disability and autistic people (LeDeR) programme to better understand the needs of people with a learning disability or autistic people and to improve future services.

The form should take around 15 minutes to complete.

Before you start

The form will ask you to provide details about:

  • The person who has died
  • The circumstances of the death
  • Your relationship to the person who has died
  • Your contact details

Please give as much information as you can in the notification form, this will help us to identify the person who has died.

The national data opt-out is a service that allows patients to opt out of their confidential patient information being used for research and planning. LeDeR complies with the national data opt out policy. Where a notification is made to LeDeR and the person had opted out of having the use of their data shared under the national data opt out policy, then the notification will not process to a LeDeR review.
